This is a form used during the raising of a Company to collect votes for the various ranks to be assigned. The form seems to indicate a tally for votes by the soldiers for Captain, however, Milton Mills was already the Captain. The soldier with the most votes, William Jenkins, entered the regiment as a Corporal. The specific purpose of this vote is unclear. Here are the ranks at which the listed soldiers were mustered into the 16th Ohio on September 13, 1861:

John B Pausler - Private
William H. Jenkins - Corporal
John Wesley Ogle - Sergeant
Charles H Roney - 1st Sergeant
Thomas S. Bagent - Private
Note that Thomas Bagent was later listed as having deserted on November 16, 1862. All other soldiers listed survived the war.
This document was among the papers of Maj. Milton Mills and was handed down through his family.
